Our tissue viability teams provides assessment and treatment for patients with complex non-healing wounds including pressure ulcers, leg ulcers, fungating wounds and post-operative wounds.

The service also provides advice to other healthcare professionals caring for patients with complex wounds and offers education programmes, advice on larval therapy, topical negative pressure and the management of lower limb primary lymphoedema.

Our tissue viability service in Brent aims to help those over the age of 16 and living in Brent, look after complex and problematic wounds. We particularly focus on preventing and treating pressure ulcers and leg ulcers.

We work closely with you, your GP, district nurses, and other healthcare professionals to provide you with support and high quality care.
 

Who we see

If you are referred to our tissue viability service, one of our specialist nurses will look at your wound. They will advise you and professionals involved in your care about how to look after it.

The tissue viability nurse may see you only once, or they may arrange a follow up visit depending on what you need.

If you need more support or advice after you have been seen by our service, your nurse or doctor can refer you again. You can also contact the service directly if you have any problems or concerns in regards to your wound.

Our teams can offer care and advice to patients with a range of wounds and injuries including:

  • Advice on the most appropriate wound dressing products
  • Joint visits with other health professionals to see patients
  • Advice on further assessment of slow healing wounds
  • Advice on pressure reducing equipment
  • Advice including how to prevent ulcer occurrence and reoccurrence

Inclusion 

  • Over 18 years of age
  • Registered with a GP in Hammersmith & Fulham, Kensington and Chelsea and Westminster

Exclusion 

  • Below 18 years of age
  • Patient that are not registered with a GP in Central London, Hammersmith & Fulham, Kensington & Chelsea

The Tissue Viability Service supports patients with non-healing wounds to access specialist nurse-led assessment and treatment. 

Patients accepted will receive a comprehensive assessment and treatment plan, which will either be delivered in a clinic, by the patient or carer in their own home or the plan will be shared with the healthcare professionals delivering patient care. 

The majority of service provision is delivered in clinics based in health centres. Domiciliary visits will be provided if the patient is housebound and is receiving care from the district nurses. This does not apply to nursing homes where the expectation is the registered nurses provide the routine/regular dressing changes. 

All patients for home visits must be referred to the District Nursing Service in the first instance.

Patients are referred to the service via healthcare professionals. All referrals for will be triaged within 24 hours of being received (Monday to Friday 8-4pm), excluding bank or public holidays and weekends.

Eligibility Criteria

Inclusions

  • registered with a Barnet GP
  • multiple category 2, single category 3 and 4 pressure ulcers
  • deteriorating pressure ulcer
  • moisture-associated skin damage
  • leg ulcer
  • non-healing wound
  • fungating wound
  • surgical wound
  • non-healing surgical wound
  • vulnerable skin
  • negative-pressure wound therapy
  • complex and/or hard to heal wounds, (including patients needing negative pressure wound therapy)*
  • leg ulcer patients who can attend a tissue viability leg ulcer clinic until ulcers healed which may include supported self-care
  • those requiring support and advice regarding pressure relieving equipment and wound management. 

Special considerations

  • In line with CLCH trust policy, patients who are non-concordant with the recommended treatment plan will be discharged from the service.
