PACE is an early supported discharge service.

The PACE team provide a five day post-acute care service to patients who are medically stable and no longer require 24 hours care although they have on-going nursing and medical monitoring needs that can be safely met outside of the hospital environment.

Patients get medical support within their own home which is mapped out by the hospital consultant.


Referral information

Case finders at Royal Free Hospital and Barnet General Hospital send patients out to the PACE teams based in Barnet, Camden, Enfield and Hertfordshire.
 
Hospital consultants refer to the case finders who then assess the patients at Royal Free Hospital and Barnet General Hospital only.

Our case finders are on duty 8am – 6pm Monday to Friday and weekends from 9am – 5pm at Royal Free Hospital and Barnet General Hospital.

Our community team works 8am – 10pm, seven days a week. Patients are seen on discharge in their own home or residential and nursing homes.

All referral are done via nhs.net.

Barnet referral information

This service cannot be referred in to from outside of the Royal Free Hospital and Barnet General Hospital until patients are assessed and accepted on to the pathway by the case finders based in the acute hospitals.

Inclusion and exclusion criteria

Inclusion:

Medically fit patients that require a further few days of care can have care provided to them in their home.

Exclusion:

Patients with mental health needs, require packages of care or deep cleaning or adaption to the home.
