Our school nursing teams help young people to make the best possible start to life by monitoring their health and promoting good health habits from an early age.

Who is our school nursing service for?

  

Our school nursing teams see children and young people between 4 to 19 years of age who attending school in Barnet, Kensington and Chelsea, Hammersmith and Fulham, and Westminster.

  

What does our school nursing service do?

  

Our school nurses have two main roles:

  • to give confidential advice, care and treatment to young people and their parents and/or carers
  • to look after and protect a young person's physical and emotional health

Our school nursing teams deliver a range of programmes including:

  • developmental screening including height, weight, vision and hearing
  • administering immunisation programmes
  • health and sex education
  • management of children with special needs
  • the Healthy Child programme (available to all pupils attending state schools)
  • safeguarding children

Most of our services are also available to offer support to parents, carers and school staff.

Our school nursing teams work across schools, health and community centres across Barnet, Hammersmith and Fulham, Kensington and Chelsea, and Westminster.

Our school nursing teams work in partnership with schools and teachers as well as a range of health & social care partners.

Children and young people can be referred by their teachers, parents and by their healthcare professionals.

Children and young people can also self-refer to our school nursing service.

Referral criteria for our school nursing service

  

Children and young people between 4 and 19 years of age who attend state schools in Barnet, Kensington and Chelsea, Hammersmith and Fulham, and Westminster.

  

What referral information does our school nursing service need?

  

Our school nursing teams require the following information to accept a referral

  • patient name
  • patient address
  • patient contact details
  • patient date of birth
  • GP details
  • name of the school they attend
  • details of any relevant health issues
  

How do you refer to our school nursing service?

  

Referrals to the school nursing service are usually made directly to the school nursing team attached to to the school.
