Our district nurses work in people's homes and the local community to help patients improve, maintain, and recover their health, cope with health problems, and achieve the best possible quality of life, whatever their disease or disability.

Every year our teams help thousands of people maintain their independence and support people return to their own homes sooner after a hospital visit.

Who is our district nursing service for?


Our district nursing service is for housebound people aged over 16 who require nursing care in their home and local community.


What does our district nursing service do?


Our district nursing teams treat a number of conditions including:

  • managing chronic long term conditions
  • caring for acutely ill patients
  • caring for post-operative patients
  • palliative care and end of life care
  • medication management
  • wound care management and advice
  • advice and management of continence problems
  • advice about healthy living
  • referral to other services

What can you expect from our district nursing service?


Our district nurse teams visit people in their homes and community to assess, treat, educate and monitor their health care needs.


Who do our district nursing teams work with?


Our district nursing teams work closely with hospitals, GPs, social services and other healthcare teams involved in our patients care.

We also work closely with physiotherapists and occupational therapists, social workers, Macmillan/Marie Curie and other specialist health care professionals to provide care to meet a wide range of health care needs.

How do you access our district nursing service?


People can be referred to our district nursing service by their GP, hospital consultant and other healthcare professionals.


We provide our district nursing services in the boroughs of:

  • Barnet
  • Hammersmith and Fulham
  • Harrow
  • Kensington and Chelsea
  • Merton
  • Westminster

Referral criteria for our district nursing service


Our district nursing teams see patients over 16 years of age who are predominantly housebound and require both routine and urgent nursing care in their homes.

Barnet,  Kensington and Chelsea, and Westminster
People have to be living in, or be registered with a GP in Barnet, Kensington and Chelsea, and Westminster in order to access our district nursing services in these boroughs.

Hammersmith and Fulham
People have to be registered with a Hammersmith and Fulham GP in order to access our district nursing services in this borough.

  • Our Kensington and Chelsea district nursing teams also provide case management services to non-house bound patients with long term conditions.
  • Case management services in Barnet, Hammersmith and Fulham, and Westminster are provided by our community matron teams.

What referral information do our district nursing teams need?


Our district nursing services require standard patient and referrer information, including NHS number, contact and address details.

We also require information about the reason for the referral, past medical history, and whether the referral is urgent, non urgent, or routine.


How do you refer to our district nursing service?


Each borough has a single access point for referrers.

Waiting times for our district nursing service


All referrals to the service are prioritised as Urgent, Non Urgent or Routine. Presently in Barnet we do not automatically acknowledge receipt of referrals unless followed up with a phone call.

The core hours of our district nursing teams are:

Day service 08.00-18.30
Evening District Nursing 18.30- 22.30
Out Of Hours  - 22.30 - 07.30

Hammersmith and Fulham:
Day service 0800-1630 (central locality), 0900-1700
Late Shift 12.00 -20.30 

Kensington and Chelsea:
Day service 08.00 -16.30
Late Shift 17.00-20.30

Day service 08.30 - 16.45
Late Shift  13.00- 21.00

Night Service (Out of Hours) 20.00 - 08.00


Discharge criteria for our district nursing service


Patients are discharged when they no longer have a nursing need, their needs can be more appropriately met by another service, or they are no longer resident in the borough.

Needs Based Assessment Form

All acute Trusts in North West London can use the Needs-Based Assessment Form to refer patients to our intermediate care services in Hammersmith & Fulham, Kensington & Chelsea, and Westminster.
Referrers can download and complete the form when referring to one of our relevant services. These are:
  • District nursing
  • Community Independence Service/ Integrated Community Teams
  • Community Rehabilitation Service and Falls
  • Community Neuro-Rehabilitation
  • Rapid Response Nursing Teams
