Service overview

Our diabetes teams provide support to adults with diabetes in a variety of community locations, and in the homes of housebound patients.

Our teams help patients control their diabetes and reduce the risk of short and long term complications by providing patients with the skills, knowledge and confidence to self-manage their diabetes effectively.

Who is our diabetes service for?

  

Our diabetes service is for people with diabetes who are over the age of 18, who are not pregnant, and are registered with a GP in the boroughs of Barnet, Hammersmith and Fulham, Kensington and Chelsea or Westminster. Our Hammersmith and Fulham diabetes service also see people that live in their borough but are registered with a GP elsewhere.

  • Type 1 diabetes (Barnet and Hammersmith and Fulham only)
  • Type 2 diabetes (in all four boroughs)
  

What does our diabetes service do?

  
  • Clinics run by a multidisciplinary team, and
  • Education initiatives.

Our clinics provide: 

  • management of insulin
  • care for patients with cardiovascular disease and diabetes
  • care for patients with diabetes related eye disease (Hammersmith and Fulham only)
  • care for people with kidney disease stage 3
  • dietetic advice

We can refer onto:

  • podiatry (foot health)
  • retinal screening
  • secondary care

We also provide home visits and support to homeless people (except Barnet).

Our education initiatives provide:

  • X-PERT & X-POD structured education programmes (except Barnet).
  • X-PERT insulin and Arabic speaking programmes (except Barnet).
  • Diabetes awareness talks to community groups
  

Where do we provide our diabetes service?

  

Our clinics are located in a variety of community locations across Barnet, Hammersmith and Fulham, Kensington and Chelsea (download factsheet here) and Westminster.

We also provide care in residential nursing homes as well as people's homes where they are housebound.

  

Who do our diabetes teams work with?

  

Our diabetes teams work closely with the following people:

  • GPs
  • social services
  • learning disabilities teams
  • mental health teams
  • other healthcare professionals involved in a patient's healthcare

In many cases we also work closely with local diabetes support groups. We can also book interpreting services to accompany patients during the consultation.

  

What can you expect from our diabetes service?

  

Patients can see a nurse, dietitian and consultant in one appointment to make the most of their clinic visit. 

We also offer joint appointments at home with the dietitian, specialist nurse and district nurse so that we can work together to ensure best care and advice possible to our patients

We offer group education and one to one educational support.

Our diabetes teams are made up of the following specialists:

  • Diabetes specialist nurse
  • Diabetes dietician (Barnet, Kensington and Chelsea, and Westminster only)
  • Retinal screener (Hammersmith and Fulham, and Westminster only)
  • Diabetes specialist community matron (Hammersmith and Fulham only)
  • Consultant
  • Nurse consultant in diabetes
  • Podiatrists
  • Education assistant (Kensington and Chelsea only)
  • GPs with special interest in Diabetes (Barnet only)

  

How do you access our diabetes services?

  

Patients can be referred by your GP, hospital consultant or other healthcare professional.

In some cases patients and their carers are able to self-refer into our diabetes services, including our group education services such as X-pert.

We treat all aspects of diabetes including the psychological side and staff are trained in cognitive behavioural therapy.

Referral criteria for our diabetes services

Our teams require the following information to triage a patient referral:

  • most recent blood glucose
  • reason for referral
  • active problems
  • significant past problems
  • current medication
  • relevant past medication
  • known allergies
  • whether the referral is urgent, non urgent, or routine

Discharge criteria for our diabetes services

  

Patients can be discharged back into the care of their GP in the following circumstances:

  • completion of a package of care
  • patient request
  • 1 or more DNAs (missed appointments)
  • 2 cancellations
  • patient transferred to secondary care
