Service overview

  

In Kensington & Chelsea we provide:

  • Early Supported Discharge for Stroke
  • Community Rehabilitation
  • Stroke Reviews
  • Stroke Support Worker
  • Dysphasia Support Group
  • Peer Mentoring Scheme
  

Early supported discharge

  

We provide early intensive rehabilitation for people with mild to moderate stroke, at the point of hospital discharge, for up to 6 weeks at the same intensity as they would receive in hospital.  This includes access to a range of clinical specialities including:

  • physiotherapy
  • occupational therapy
  • speech and language therapy
  • psychology
  • orthotics
  • rehabilitation assistants

There is also access to a range of community services including dietetics and podiatry. We work in partnership with other community support services for people with stroke.

If further rehabilitation is required at the end of six weeks, clients transfer to the community neuro rehabilitation service.

Services are provided in people's homes, residential homes and in other community locations. We work with clients and their carers towards their individually agreed goals and provide early assessment, treatment and advice regarding:

  • Indoor/outdoor walking practice
  • Promoting independence with daily activities, e.g. meal preparation, bathing
  • Swallowing, eating, drinking
  • Communication - speaking,(articulation or voice), understanding, reading, writing
  • Cognitive - e.g. memory and attention
  • Emotional difficulties
  • Fatigue management
  • Work, education and leisure
  • Education and support to carers
  

Stroke reviews and support

  

Stroke Reviews
We carry out holistic Stroke Reviews with stroke survivors and their carers' at 6 and 12 months, following hospital discharge, to identify any unmet needs. We enable clients to seek solutions as well as refer or signpost to other services as required.

Stroke Support Service
This service is open to all people who have had a stroke and their carers' and is available in hospital, client homes', including residential care homes, and other community locations. The service is provided for up to one year following stroke and includes the provision of information, advice, advocacy, emotional support and guidance to other services.

Dysphasia Support
This service is for people with communication difficulties following stroke, for example aphasia. We provide a dysphasia support group that runs weekly on a Wednesday morning at the Community Hall, Notting Hill Methodist Church.

Peer Mentoring Scheme
We facilitate a peer mentoring scheme for stroke survivors to provide support to others who have experienced stroke, who are at an earlier stage in their recovery.

Referral criteria - early supported discharge


Referrals to the service can be made by hospital therapists, consultants and other health and social care professionals.

Stroke Reviews
Referrals can be made by GPs, hospital services, and health and social care professionals. We also accept self-referrals from stroke survivors and their carers.

Stroke Support Service
Referrals can be made by GPs, hospital services, health and social care professionals, and voluntary agencies. We also accept self-referrals from stroke survivors and their carers.

Dysphasia support
We accept self-referrals from stroke survivors and their carers, from GPs, hospital services, health and social care professionals, and voluntary agencies.
 
Core hours
The core hours for our community stroke services are Monday to Friday, 8.30 am to 17.00pm.

Discharge criteria
Patients can be discharged when they have reached their full rehabilitation potential, have achieved their rehabilitation goals, or where there is no clinical benefit to continued rehabilitation.

Patients can also be discharged back to the care of their GP if they fail to attend appointments.
