About Us

ICT-street -250CLCH provides community health services to around a million people across London and Hertfordshire.

Every day, our professionals provide high quality healthcare in people's homes or at convenient  local clinics, helping them to stay well, manage their own health with the right support and avoid unnecessary trips to, or long stays in hospital.

We support our patients at every stage of their lives; providing health visiting for new-born babies through to community nursing, stroke rehabilitation and palliative care for people towards the endof their lives. We also help to ease the pressure on Accident and Emergency departments by offering walk-in and urgent care centres to treat people with minor injuries.

Our vision is to deliver: Great care closer to home

Our mission is: Working together to give children a better start and adults greater independence
